The literature suggests that both arthroscopic posterior Bankart repair (APB) and posterior bone block (PBB) are effective procedures in the short to medium term, although recurrence and revision rates do not appear to be negligible. However, fewer studies, especially comparative ones, are available regarding the long-term outcomes of these procedures.   In conclusion, at a mean follow-up of 5 years for APB and 12 years for PBB, the APB group had a similar recurrence rate but fewer complications and revisions compared with the PBB group. Both techniques resulted in good to excellent clinical and functional outcomes, as well as high satisfaction and RTS rates.   Click here to read the article.

Previous studies have shown that arthroscopic Bankart repair (ABR) for the treatment of anterior shoulder instability (ASI) may lead to high rates of instability recurrence and revision surgery at 10-year follow-up, but data on 20-year postoperative outcomes are scarce.   In conclusion, about 1 in 3 patients reported instability recurrence or redislocations, and 1 in 5 underwent further surgery. In patients who did not undergo further surgery, good to excellent shoulder function as well as low pain and instability levels were observed at a minimum 20 years after ABR. The presence of inferior glenohumeral laxity was associated with a higher risk for subjective reinstability, and the use of fewer anchors was associated with redislocations.   Click here to read the article.

The high rates of failures and reoperations (up to 50%) after arthroscopic posterior Bankart repair (APBR) remain a concern.   In conclusion, in patients with recurrent posterior instability, the presence of posterior glenoid erosion was the main risk factor for failure after APBR. The femoral attachment of the anterolateral ligament (ALL) reported by anatomic studies is posterior and proximal to the lateral femoral epicondyle.   In conclusion, independent ALL reconstruction while performing a percutaneous technique enabled anatomic positioning of the ALL graft in 79% of cases. In recent years, anterior cruciate ligament (ACL) injuries in children and adolescents (age ≤18 years) have been increasing, and the quadriceps tendon (QT) autograft has been gaining popularity for ACL reconstruction. However, there is no consensus regarding the graft choice for ACL reconstruction in these young patients. There has been a paucity of literature comparing the functional outcomes of hamstring tendon (HT) and QT autografts for ACL reconstruction in patients aged ≤18 years.   In conclusion, a QT autograft for ACL reconstruction led to similar clinical outcomes, revision rates, and sports participation compared with an HT autograft in pediatric and adolescent patients.   Click here to read the article.

Despite advancements in surgical techniques, the recurrence rate of anterior cruciate ligament (ACL) rupture after reconstruction remains between 2% and 20%. An increased posterior tibial slope (PTS) is associated with higher ACL rupture and reconstruction failure rates.   In conclusion, ACW-HTO combined with ACL reconstruction shows promising long-term outcomes, significantly reducing PTS and improving knee stability and function with an acceptable complication rate.   Click here to read the article.

Focal cartilage lesions are commonly associated with anterior cruciate ligament injuries. Emerging evidence suggests that anterior cruciate ligament (ACL) ruptures can restore ACL fiber continuity. The relationship between ACL continuity on magnetic resonance imaging (MRI) (sign of ACL healing) and outcomes >5 years after an acute ACL rupture has not been investigated.   ACL continuity on 5-year MRI may be associated with worse patient-reported outcomes at 11 years after an ACL injury compared with early or delayed ACLR.   Click here to read the article.

Sagittal-plane slope-reducing high tibial osteotomy (HTO) can reduce the risk of anterior cruciate ligament (ACL) injury in knees with a high posterior tibial slope. The biomechanical effect of slope-reducing HTO on tibiofemoral kinematics and force carried by the ACL remains less well understood.   In conclusion, slope-reducing HTO decreased ACL force linearly by reducing ATT with compression and also by decreasing coupled ITR with combined compression and valgus.   Click here to read the article.
